You will need to provide consent by Monday 20 April by completing the consent form here. You will need to enter the school code which is DT137349.

If your child is in another year group and has missed these vaccinations previously, catch-up doses are also available.

If you miss the deadline, you can still complete the form, and your child will be offered an appointment at a later clinic or during a return school visit.

If your child cannot attend the school session, appointments can be booked at local clinics (availability updated regularly).
